We acknowledge every referral, and we tell you whether we were able to serve the family — including when we could not, and why.

Please do not email protected health information. Ordinary email and standard website forms are not appropriate for diagnoses, evaluations, treatment records, or identifying clinical detail. Use the secure intake pathway below. This website does not collect clinical records through a WordPress form.

Services accepted

ABA TherapyFlorida and North Carolina. Center-based in West Palm Beach for ages 2 to 6; home, school, daycare, and community settings in North Carolina.
Occupational TherapyPrivate pay, based in Florida. Not billed to insurance.
Mental Health CounselingPrivate pay, based in Florida. Not billed to insurance.
Not offeredSpeech therapy and speech-language pathology are not provided by Look Up Therapy. Please route those referrals elsewhere.

Referral checklist

Referrals move fastest when these are available. Send them only through the secure pathway.

  • Caregiver name and preferred contact method
  • Child's age and the county where services are needed
  • Service being requested
  • Diagnostic evaluation or report, where one exists
  • IEP, 504 plan, or school behavior plan, if applicable
  • Insurance or funding information
  • Signed release of information where required
  • Any urgency or safety considerations, stated generally

What happens after a referral

  1. We acknowledge receipt

    Our intake team confirms the referral arrived and identifies anything missing.

  2. We contact the family

    The caregiver is reached directly to begin intake and answer questions.

  3. We verify fit and funding

    Clinical fit, geographic coverage, and funding are reviewed before commitments are made.

  4. We close the loop with you

    With appropriate consent, we tell you whether the family engaged and whether we were able to serve them. If we could not, we say why.
